Dup15q Resources and Support Network Pilot Program

Dup15q Alliance is proud to partner with the Child Neurology Foundation to link families to the Neurology Social Services Network, a FREE PROGRAM. This is a short-term pilot program.  We aim to connect families with resources and programs that may be available to them. In some cases, we may be able to refer families to businesses and organizations located right in their area. Although we can’t guarantee that we’ll be able to match families with the requested support, we will exhaust the resources available to us. This is a short-term pilot program.

We hope you will share a little data with us. This information will assist in identifying all possible support for which your family may be eligible. And, with your permission, your de-identified data can be aggregated with the Child Neurology Foundation’s figures to highlight the needs of the greater neurology community to share with decision-makers locally and nationally.

We ask that the person who is receiving the service being requested is the ‘client’. If this is for the individual with dup15q, the client would be the individual’s name/DOB and the contact info is the parent.

By completing the form below, you’ll be entered into our secure, coordinated network of community resources, powered by Unite Us, that is available to help. A specialist will contact you after you submit the form.

Thank you for taking the time to fill out our form. Our team will reach out to you within 5-7 days.
